What is ECMO?

Extracorporeal Membrane Oxygenation (ECMO) is a form of life support used for patients with life-threatening heart and/or lung problems. ECMO provides rest to these organs and helps them to recover by doing the work of the heart and lungs.

In extracorporeal membrane oxygenation (ECMO), blood is pumped outside of the body to a heart-lung machine that removes carbon dioxide and sends oxygen-filled blood, back to the body. Blood flows from the right side of the heart to the membrane oxygenator in the heart-lung machine, and then is rewarmed and sent back to the body. This method allows the blood to “bypass” the heart and lungs, allowing these organs to rest and heal. ECMO is used in critical care situations when the heart and/or lungs need help so that they can heal. Depending on the condition, ECMO can be used for a few days to a few weeks.

The doctor will insert a thin, flexible tube (cannula) into a vein to draw out blood and a second tube into either a vein or an artery to return warmed blood with oxygen to your body. Patient will receive other medications, including sedation, to make the patient comfortable while receiving ECMO.

Types of ECMO

Veno Arterial: involve s venous blood from patients blood being access from the large central vein and return to major artery it passes through the oxygentrator resulting in increase oxygen levels in the body

Veno Venous: involves from the patients large central vein blood passes through oxygentrator and enters into another large vein in the body. Used mainly insevere respiratory failures, lung injury (mechanical ventilator induced trauma) and ARDS.

When is ECMO?

Some heart (cardiac) conditions in which ECMO may be used include:

Heart attack (acute myocardial infarction).
Heart muscle disease (decompensated cardiomyopathy).
Inflammation of the heart muscle (myocarditis).
Life-threatening response to infection (sepsis).
Post-transplant complications.
Shock caused by the heart not pumping enough blood (cardiogenic shock).
Bridge to Heart and Lung transplant.
Pre or post cardiac surgery depending on clinical condition.

Some lung (pulmonary) conditions in which ECMO may be used include:

Acute respiratory distress syndrome (ARDS)
Blockage in a pulmonary artery in the lungs (pulmonary embolism).
Coronavirus disease 2019 (COVID-19).
Defect in the diaphragm (congenital diaphragmatic hernia).
Foetus inhales waste products in the womb (meconium aspiration).
Flu (influenza).
High blood pressure in the lungs (pulmonary hypertension).
Severe Pneumonia
Respiratory failure
Lung Trauma
Poisoning leading to lung/heart failure.
