
20 months old male baby was brought to PICU with severe breathlessness and seizures following accidental ingestion of Turpentine oil.
He was not able to maintain oxygen in his body even while on HOFV (advanced mode mechanical ventilation) and NO (nitric Oxide), his lungs were failing rapidly despite maximum support from the conventional method of treatment.
To maintain oxygenation in the body, and to allow his lungs take rest and recover, the baby was placed on VV ECMO (femoral vein to Internal Juglar vein).
In a few days he had a problem with his lung pressure which became very high (PAH) and resulted in decreased right heart function which affected his kidney function also.
He required CRRT (continuous renal replacement therapy) on VV ECMO and medication to maintain normal blood pressure.
After 3 weeks of the waxing-waning course, his lungs started showing signs of improvement, his clinical condition stabilized, and he was weaned off ECMO and shifted over to conventional ventilation on the 21st day of admission after which he was successfully extubated. 45th day, he was discharged with no clinical sequelae and last we heard, he is regularly attending his school.
